Med spa local SEO: how GBP entity signals, schema and PageSpeed turn a lunch-break search into a booked consult

The injector with the steadiest hands does not automatically get the patient. The med spa Google can read as a medical entity, a nervous first-timer can trust in five seconds and a phone can load in two does.

Med spa local SEO: how GBP entity signals, schema and PageSpeed turn a lunch-break search into a booked consult

Why the better injector loses the patient who just got her headshots back

It is 12:35 on a Wednesday. A realtor opens the proof gallery from her brokerage headshot session, zooms in on the two lines between her brows, and types "botox near me" before she finishes her salad. She has never had a treatment. In four minutes she scans three Map Pack results, reads two reviews that say "natural" and "not frozen", checks whether the person holding the syringe is a nurse practitioner or a rented room in a hair salon, finds a price per unit on one site and "call for pricing" on the other two, and books a consult through the one with a working booking button. She never spoke to anyone.

The med spa across town with the more experienced NP injector may not have appeared at all. Its profile still listed the old strip-mall address, its site loaded a full-screen Instagram grid before the phone number, and its pricing lived in a PDF menu. This article is about closing that gap, one entity signal at a time.

The short version for aesthetic practices

Google ranks local results on relevance, distance and prominence. Distance is set by where the patient is standing, so the winnable fight is relevance, prominence and conversion. At Digital Domination we build med spa local SEO as a trinity with a measurement loop around it:

  • Google Business Profile entity signals establish that this is a medical aesthetics practice, who injects, which devices are in the building and what a treatment costs.
  • Schema turns the medical director, injectors, procedures, per-unit offers and locations into a machine-readable graph with permanent identifiers.
  • PageSpeed makes sure a first-timer on a lunch break, a Google crawler and an AI agent can all reach the booking button immediately.
  • Search Console, GA4 and the booking system show which searches produced consults and which consults produced treatment revenue.

GBP entity signals for a med spa

The primary category should be Medical spa. Secondary categories belong only to services you deliver under the right license: Skin care clinic, Laser hair removal service, Weight loss service or Wellness center. Choosing Plastic surgeon or Dermatologist because it sounds clinical is a fast route to a suspended profile and a state board complaint. Services carry the words patients type: neuromodulators (Botox, Dysport, Xeomin, Jeuveau), fillers by area (lips, cheeks, under-eye, jawline), Sculptra, Kybella, RF microneedling, IPL photofacial, laser hair removal, chemical peels, HydraFacial, body contouring, GLP-1 weight management and IV therapy. Each service gets a description and a starting price, and each must exist as a page on the site. The remaining fields do more work in this niche than most owners expect:

  • Photos: real treatment rooms, the actual device brands, injectors in scrubs, and consented before-and-after pairs. Stock models with airbrushed skin read as a franchise to patients and to Google.
  • Attributes: Online appointments, Women-owned, LGBTQ+ friendly, Wheelchair accessible entrance, and Appointment required if walk-ins are not seen.
  • Hours: evening and Saturday injector days are an edge, so publish them and set special hours for holiday weeks when neuromodulator demand spikes.
  • Booking link: point Book online at your Boulevard, Mangomint, Vagaro, Zenoti or Aesthetic Record page, not the homepage.
  • Per-location landing pages: each profile links to its own page with that location's injectors, devices, parking and a UTM-tagged booking button.
  • Q&A: seed the front-desk questions, including who performs the good faith exam and whether the consult is free.
GBP entity signals for Med Spas: categories, services, location, hours and reviews feed the Google Business Profile, producing relevance and prominence, then Maps visibility.
Every Google Business Profile field is an entity signal. Complete and consistent beats clever. (tap to enlarge)

Authority above the fold: who is holding the syringe

A med spa sells a medical procedure in a spa setting, so the first screen has to resolve the safety question before it sells the result. Patients want an MD or DO named as medical director, an RN, NP or PA injector with named training, and a practice that follows the state's good faith exam rules. An example authority strip, with placeholder figures only, might read: Physician-owned, NP and RN injectors, Allergan Medical Institute trained, X years in aesthetics, 4.9 stars from X reviews, complimentary consults. The proof behind it belongs on the page and in the bios:

  • The medical director's name, degree, state license and a link to the state medical board lookup.
  • Each injector's credential (RN, NP, PA), license number, years injecting and a headshot taken in your rooms.
  • Manufacturer training and status: Allergan Medical Institute, Galderma GAIN, Merz, and any partner tier stated exactly as the manufacturer states it.
  • Device brands and models by name (Candela GentleMax Pro, InMode Morpheus8, Sciton, Cynosure), because patients search for the device.
  • American Med Spa Association membership, HIPAA-compliant intake, single-use product policy and emergency protocols.
  • Before-and-after galleries filtered by treatment, with consistent lighting, units or syringes used, and the interval between photos.
Authority plus CRO for Med Spas: credentials, reviews and proof feed the above-the-fold landing page alongside pricing and availability, producing trust and the primary call to action.
Authority is what the first screen proves before anyone scrolls. Trust is what it earns. (tap to enlarge)

Review velocity when patients do not want to be seen

Aesthetic patients are often private about treatment, which makes a steady review cadence harder and more valuable than in most local niches. Ask at the two-week follow-up, when swelling is gone and the result has settled, with a text that lands on the Google review form in one tap. Never incentivize reviews, and never reply in a way that confirms a named person received a treatment; a HIPAA-safe reply thanks them for visiting without repeating clinical detail.

Review language is entity fuel. When a patient writes that Jenna did her lip filler in Scottsdale and it looked natural with no bruising, Google connects the injector, the service, the city and a quality attribute in one sentence. Summarize combined Google and RealSelf ratings above the fold, then expand by treatment so a Morpheus8 shopper sees Morpheus8 reviews. Watch the negative pattern too: complaints about package pressure or a surprise per-unit total are how the first-timer at the top of this article gets lost.

The review velocity flywheel: a great client experience, a review request, a fresh review, a helpful response, more trust, more clients, repeating.
Velocity beats volume. A steady stream of recent, specific reviews outperforms a large stale count. (tap to enlarge)

Schema architecture for a medical spa

Schema.org does not ship a dedicated MedicalSpa type, so the honest approach is a HealthAndBeautyBusiness node that also declares MedicalBusiness as an additionalType, a Physician node for the medical director and Person nodes for every injector. Give the practice, each location, each injector and each treatment one permanent @id (yourdomain.com/#practice, yourdomain.com/botox/#service) and reference it from every page, so the Botox page, the injector bio and the location page all resolve to the same entity. Use sameAs to point at the GBP, RealSelf, the Allergan and Galderma provider locators, the AmSpa directory and the state board lookup. Every fact in the schema must be visible on the page. The graph, as node and type pairs:

  • The practice: HealthAndBeautyBusiness with additionalType MedicalBusiness, permanent @id, name, address, geo, telephone, openingHoursSpecification and image.
  • Each location: its own HealthAndBeautyBusiness node with parentOrganization pointing at the practice @id.
  • Medical director: Physician with medicalSpecialty, hasCredential (EducationalOccupationalCredential) and an identifier PropertyValue for the license number.
  • Injectors: Person with jobTitle, worksFor, hasCredential, knowsAbout and sameAs to the state board lookup.
  • Each treatment: Service (or MedicalProcedure where it fits) with provider, areaServed and its own permanent @id.
  • Pricing: Offer with a PriceSpecification per unit, per syringe, per session or per package, plus priceCurrency.
  • Proof and plumbing: ImageObject for consented before-and-after pairs, VideoObject for explainers, Review and AggregateRating only from reviews shown on the page, plus WebSite, WebPage, BreadcrumbList and FAQPage.

PageSpeed for a site built like an Instagram feed

Med spa sites fail speed tests for predictable reasons: an autoplay hero video of a laser handpiece, a live Instagram grid, a booking widget that loads its whole app on the homepage, before-and-after sliders shipping full-resolution JPEGs, a discount pop-up and three ad pixels. Google's good thresholds for Core Web Vitals are LCP 2.5 seconds, INP 200 milliseconds and CLS 0.1, measured at the 75th percentile of field data. At Digital Domination we target a PageSpeed Insights performance score of 90 or higher and a 2/2 result on the agentic-browsing check, because an AI agent booking a consult hits the same walls a slow phone does. The fixes with the largest payoff here:

  • Replace the autoplay hero video with a preloaded WebP or AVIF still of your actual room and injector, and never lazy-load that image.
  • Load the booking widget on click or on the booking page only; keep a lightweight Book now button and tap-to-call number in the sticky mobile bar.
  • Serve galleries as responsive, compressed images with reserved dimensions so the page does not jump while a patient compares photos.
  • Swap the live Instagram embed for six static images that link out, and delay chat, heatmap and non-essential pixels until after interaction.
  • Self-host and subset the two brand fonts, drop unused theme CSS, and put the site behind a CDN with caching and modern compression.
  • Kill the entry pop-up on mobile and put the new-patient offer in the hero copy, where it also helps conversion.
PageSpeed to conversion: optimized images, lean code and caching produce a fast usable page, clear content, an instant call to action and more conversions. Core Web Vitals targets LCP 2.5 seconds, INP 200 milliseconds, CLS 0.1.
PageSpeed is conversion infrastructure. The score matters less than whether the phone number is tappable in the first second. (tap to enlarge)

Per-unit and per-syringe pricing for AI Overviews

Pricing is the most common question AI systems are asked about med spas and the one most sites refuse to answer. "Call for pricing" leaves ChatGPT, Gemini and Google's AI Overviews to quote the competitor who published a menu. Publish the model even when the total varies: Botox and Dysport by the unit with a range, fillers by the syringe and area, laser hair removal by area and package, RF microneedling by session with the recommended series, GLP-1 programs by month, and memberships with what they include. Explain why totals vary (units needed, syringes, skin type for laser settings), state whether the consult is complimentary, and list Alle and Aspire rewards participation. Put that language on each treatment page, in the FAQ and in GBP services, and mirror it in the Offer schema so the numbers a model reads match the numbers a patient sees.

Content that walks a first-timer from curiosity to consult

A first-time aesthetic patient researches for weeks, and the practice that answers her questions in plain language usually gets the appointment. Each treatment page should cover who is a good candidate, how it works, what the appointment feels like, typical units or sessions, when results appear and how long they last, downtime and bruising, contraindications such as pregnancy or recent dental work, aftercare, cost and who performs it. Add state detail where it exists, such as who may inject and what a good faith exam is in your state. Seasonal guides earn their keep: fall and winter for laser hair removal and IPL when sun exposure drops, spring for pre-wedding neuromodulator timing, November for holiday-party timelines. Every article carries a named injector author, a medical director reviewer line, dates, and internal links to the treatment, the bio and the booking page.

Content that converts for Med Spas: a customer question leads to the service page, which proves local expertise, the provider's authority and real results, earning trust and the booking.
Every money page answers the question the customer arrived with, then proves it can be trusted with the answer. (tap to enlarge)

Search Console, GA4 and the booking system as one loop

Search Console shows the queries that surface the practice, the pages Google associates with them, click-through rate, indexing issues, Core Web Vitals and structured-data errors. GA4 shows what happened next: Book now clicks, booking starts and completions, tap-to-call, consult form fills, gallery views and which treatment pages convert. The booking or EMR system closes the loop with consult show rate, treatment conversion and membership sign-ups. A page ranking for "lip filler near me" with impressions but few clicks needs a better title, opening copy and review proof; clicks with no bookings call for a speed, pricing and trust review; consults that book but do not convert send you back to the offer, the injector match and the front-desk script. Annotate every deployment date, change one focused group of things at a time, and compare bookings and treatment revenue, not just rankings.

Engagement that matters: GBP, social and organic visitors land on the page, then read, watch, scroll and click until intent is satisfied by a call, form or booking. Ten seconds and a call is a win; six minutes and an exit is not.
Measure what the visitor did, not how long they stayed. A ten-second visit that ends in a call is the win. (tap to enlarge)

Instagram, TikTok and brand mentions as entity corroboration

Aesthetics is the one local niche where social is a primary discovery channel rather than a supporting one. The job is to make every post reinforce the same entity: the same injector names, treatment vocabulary, location and booking link. A TikTok on what 20 units does to elevens, a consented before-and-after reel, a live Q&A with the NP and a GBP post about a new device should all point at the matching treatment page. Google's Search Console now reports supported social and video platform properties, so that content is measurable alongside organic search. Off site, RealSelf, the manufacturer provider locators, the AmSpa directory, state board lookups, local lifestyle press and wedding vendor listings corroborate the entity. Axios reported in August 2026 that Reddit's share of visible ChatGPT Search citations fell roughly 86 percent, yet OpenAI has a Reddit data partnership, so a genuine mention in a city subreddit or skincare community still carries context even when it is not the cited link. Do not depend on any one platform staying favored.

A 90-day roadmap for a med spa

The order matters here: credentials and pricing first, because every other signal depends on the practice looking legitimate. Then speed, then the review engine, then the optimization that compounds. The plan we run:

  • Days 1-14, baseline: GeoGrid scans for botox, lip filler, laser hair removal and med spa near me; audit every GBP field and service; inventory injector credentials, devices and consented photos; pull current schema and Core Web Vitals; set GA4 booking events and UTM tags on every booking link.
  • Days 15-30, foundation: fix categories and services, publish injector and medical director bios with license lookups, put per-unit and per-syringe pricing on every treatment page, deploy the entity graph with permanent @ids, remove the autoplay video and defer the booking widget.
  • Days 31-60, relevance and proof: build treatment pages with candidate, cost and downtime sections, publish seasonal timing guides, launch the two-week review request, align RealSelf and manufacturer locator profiles, and start weekly injector-led short video.
  • Days 61-90, optimize: mine Search Console for high-impression, low-click treatment queries, find high-traffic, low-booking pages in GA4, compare consult show rate and treatment conversion by source, recheck Core Web Vitals and agentic usability, and remeasure the GeoGrid.

One medical entity, corroborated everywhere

The target is not a polished profile, a pretty website or a busy Instagram account in isolation. It is one clearly defined medical aesthetics entity whose medical director, injectors, credentials, devices, treatments, prices, locations and consented results say the same thing on Google, on the website, in the schema, in the reviews, on social and in the directories that vouch for it. Then Search Console, GA4, the booking system and GeoGrid data make that entity more relevant, prominent and profitable every month. If the realtor on her lunch break is booking somewhere else, start by finding out where you actually rank. Digital Domination's free GeoGrid scan checks 169 points across your market so you can see the whole map, not just the view from your front desk.

Frequently Asked Questions

What should a med spa use as its primary Google Business Profile category?

Medical spa is the correct primary category for almost every practice. Add secondary categories only for services delivered under the right license, such as Skin care clinic, Laser hair removal service or Weight loss service. Do not choose Plastic surgeon or Dermatologist unless a board-certified physician in that specialty practices on site.

Should a med spa publish Botox and filler prices online?

Yes. Publish the pricing model even when totals vary: neuromodulators by the unit with a range, fillers by the syringe and area, lasers by area and package, and memberships with inclusions. AI systems and first-time patients both skip practices that say call for pricing, and the published numbers should match GBP services and the Offer schema exactly.

Is there a schema.org type for a medical spa?

Schema.org does not have a dedicated MedicalSpa type. Use a HealthAndBeautyBusiness node that also declares MedicalBusiness as an additionalType, a Physician node for the medical director, Person nodes for injectors, and Service or MedicalProcedure nodes for treatments. Give each entity a permanent @id and reference it from every page.

How do we get reviews when patients are private about treatments?

Ask at the two-week follow-up, once results have settled, with a one-tap text link to the Google review form. Never incentivize reviews, and reply without confirming that a named person received a specific treatment. A steady cadence of honest reviews that mention the injector and treatment matters more than a burst of vague five-star ratings.

Does schema directly improve med spa rankings?

Schema is not a guaranteed ranking boost on its own. It helps Google and AI systems understand who the medical director is, who injects, what each treatment costs and how locations connect, and it can make pages eligible for supported result features. Its value depends on describing facts that are visible on the page.

Do Instagram and TikTok posts help a med spa rank in Google Maps?

Google does not confirm that social engagement is a direct local ranking factor. In aesthetics, though, social is a primary discovery channel, and consistent posting drives branded searches, reviews, mentions and GBP interactions that support prominence. Search Console now reports supported social and video properties, so that content can be measured alongside organic search.

What is the biggest PageSpeed problem on a typical med spa website?

Usually an autoplay hero video or live Instagram grid loading before the phone number and booking button, followed by a booking widget that loads its whole app on the homepage. Replace the video with a preloaded still, load the widget on click, and defer chat and tracking pixels. The aim is LCP under 2.5 seconds, INP under 200 milliseconds and CLS under 0.1 in field data.
